Driver’s Crash Reports (CR-2) are also available online.   The CR-2, also called a “blue form,” can be completed by a driver of a vehicle involved in a crash that is not investigated by a Frisco Police Department officer.

The driver of a motor vehicle involved in a crash not investigated by a law enforcement officer and resulting in injury to or death of any person, or damage to the property of any one person, including himself, to any apparent extent of at least one thousand dollars ($1,000), must within 10 days after such crash complete and forward CR-2 report in accordance with the instructions on the CR-2.

Many times an officer will be called to the scene of the accident but allow drivers to exchange information and advise them to complete this form.  More information on the blue form’s use is located on the first page of the form.
